Please send screenshot(s) clearly showing:
- Device Name
- Storage Capacity
- EID
- IMEI 1
- IMEI 2
If all information does not fit on one screen, multiple screenshots are acceptable.

- This device-information requirement applies only to customers who selected LG U+ Prepaid eSIM Conversion.
- LG U+ Prepaid eSIM Conversion is available on compatible iPhones only. Android devices are not supported.
- Customers purchasing a regular Data eSIM do not need to submit these device-information screenshots unless KoreaUSIM specifically requests them.
