Before You Submit

⚠️ The application form must be completed by hand.

Please print the application form and handwrite all customer information.

Do not type directly into the PDF.
Do not complete the form using a keyboard, computer, smartphone text tool, or PDF text editor.
Your name must be handwritten.
Both required signature sections must be signed by hand.
Typed names or typed / digital signatures are not accepted.

Complete all required customer fields.
Leave all carrier-use fields blank. KoreaUSIM will complete those sections.

After completing the form by hand, you may submit it as either:

A clear scan of the completed form, or
A clear smartphone photo of the completed form

⚠ Passport and ID documents are different. Passports, Alien Registration Cards, and Residence Cards must be scanned using a document scanner connected to a computer. Smartphone photos are not accepted for ID documents.

What You Need to Fill In by Hand

Please handwrite the following information:

Preferred phone number – last 4 digits
Name
Date of birth
Contact number
Passport / ARC / Residence Card number
Address in Korea
Required name and signature sections

Name

Korean nationals: Write your name in Korean by hand.
Foreign nationals: Write your full name in English by hand, exactly as shown on your passport or identification document.

Signatures

Both required signature sections must be personally handwritten by the applicant. Typed, copied, pasted, or digitally inserted names and signatures are not accepted.
